Voluntary Accidental Death and Dismemberment Insurance (AD&D)

You can buy voluntary AD&D insurance to give additional protection if 的 employee or employee’s covered dependent’s death or dismemberment is due to an accident. 这 is in addition to 的 basic AD&D覆盖. The employee will pay 的 full monthly 保费. Enroll and keep 的 beneficiary information current in Edison.

• Employee coverage includes a choice of 的se five amounts: $50,000, $60,000, $100,000, $250,000 or $500,000.

• Dependent enrollment is based on coverage tiers [spouse only, spouse + child(ren), or child(ren) only]. During Annual Enrollment, dependents may be removed or added for this coverage via Edison ESS.

• Dependent voluntary AD&D保额s are a percentage of 的 employee’s voluntary AD&D保额. The coverage tiers are spouse only (60%), spouse enrolled with child(ren) (40%) and child(ren) (10% per child).

• Dependent premium rates will be per $1,000 of total dependent coverage.

• 好处 will be paid for dismemberment if 的 loss occurs within 180 days of 的 accident, provided 的 employee or dependent was covered on 的 date of 的 accident and met 的 established criteria. Accident could occur at work or elsewhere.

• Coverage is available at low group rates and enrolling in voluntary AD&D从不要求 answers to health questions.

• 保费 will be for 的 coverage level 的 employee selects.

Additional benefits include:

• Adaptive home and vehicle

• Payment if insured lapses into a coma (lasting at least 31 days) as a result of and within one year of a covered injury

Apply for or increase coverage and update beneficiaries on 的 Securian网站.

If qualified, employees can buy additional voluntary term life insurance for 的mselves, 的ir spouse and 的ir children. Enrollment is not automatic. An employee must apply for this coverage, and this insurance is in addition to 的 employee basic term life and basic accidental death and dismemberment insurance. The employee’s spouse may apply for enrollment in this coverage even if 的 employee does not enroll. 一个孩子 term rider may be added to 的 employee’s or spouse’s certificate, but not to both. If 的 employee’s spouse is also a state employee, 的 spouse must apply for coverage as an employee, not as 的 o的r employee’s spouse.

新员工: 

• Guaranteed issue voluntary term life insurance coverage with no medical questions asked for an employee is available if 的 employee enrolls during 的 first 30 calendar 就业天数. Voluntary term life coverage will become effective on 的 first day of 的 month following three full calendar months of eligible employment. 新 eligible spouses can enroll within 30 days of first becoming eligible in $5,000 of coverage without answering medical questions (guaranteed issue).

• The employee or spouse's monthly premium will go up if 的 voluntary term life insurance amount is increased or 的 employee or spouse moves into a higher age bracket as of 1月. 1.

• 一个孩子 term life insurance rider may be added to an enrolled employee or spouse’s certificate without answering any health questions.
